A Zimbabwean citizen who was injured when a letter bomb addressed to Father Michael Lapsley exploded in Harare, Zimbabwe in April 1990. Mr Mutizwa was visiting Fr Lapsley at the time of the explosion. Fr Lapsley suffered severe injuries in this attempted killing which the Commission determined was a South African security force operation.
